University of Alberta Health Insurance Plan (UAHIP)
A minimum quantity of 1 is required per purchase. Each increment equals 1 month. You can purchase in increments of 1 month, up to a maximum of 14 months per purchase.
IMPORTANT: Payment of this fee does not guarantee enrollment in this insurance plan.This UAHIP Enrollment Request Form must be completed and submitted as part of the enrollment process.
