The listed fee covers one month of insurance for the number of family members selected.
You must purchase the total number of months required for the entire duration of your family members’ stay. Please select the appropriate quantity before proceeding to checkout.
IMPORTANT: Payment of this fee does not guarantee enrollment in this insurance plan. This UAHIP Enrollment Request Form must be completed and submitted as part of the enrollment process.
