- Appointments must be purchased at least TWO WEEKS prior to the appointment date.
- Please bring the email you receive upon purchase with you to your appointment to confirm your payment.
- If your name is different from the one on the credit card used for purchase, please inform your mask fit tester when you arrive.
Cancellation/Appointment Change Policy:
- An appointment must be cancelled at least 48 hours prior to scheduled appointment time in order to receive a refund.
- A $5.00 cancellation fee will apply to all cancellations.
- There will be no refunds for "no shows" (failing to arrive for a scheduled appointment).
- There will be no refunds for "late arrivals" (arriving 10 minutes or more after the scheduled appointment time).
- There will be no moving of appointment date without a valid reason. This must be discussed ASAP and is at the discretion of the mask fit testing team.
