A quantity of 1 equals to 1 month of coverage. You may purchase in increments of 1 month. Coverage begins on the first calendar day of the month to the last calendar day of the month.
Payment does NOT guarantee enrollment in this health insurance plan. You must complete the ELS - UAHIP Enrollment Request (Dependants) form as part of the enrollment process.
